Alprostadil: A Doctor’s Guide to This ED Treatment

I often see men in my clinic who feel a deep sense of frustration. They’ve tried the usual pills for erectile dysfunction (ED), and for one reason or another, they just aren’t the right fit. If that sounds familiar, I want you to know you’re not out of options. Today, let’s talk about a different approach, a medication called Alprostadil, which can be a very effective next step for many men.

It’s not a cure for the underlying causes of ED, but it can be a reliable and safe way to restore intimacy and confidence.

What Exactly Is Alprostadil?

Think of Alprostadil as a helper molecule that’s identical to a natural substance your own body produces, called prostaglandin E1. Its main job is to relax the walls of blood vessels, making them wider. We call this type of medication a vasodilator.

When used for ED, it increases blood flow directly into the penis. This is what helps create and maintain an erection firm enough for sexual activity. It’s a very direct and effective mechanism.

It’s important to be clear: this medication is only for those diagnosed with ED. Using it improperly, or without a doctor’s guidance, can lead to serious and even permanent damage.

Is Alprostadil the Right Next Step for You?

So, how do we decide if this is a good path for you? Usually, we consider Alprostadil when first-line treatments like oral pills (think Viagra or Cialis) haven’t worked well or have caused unpleasant side effects.

Before we even consider a prescription, it’s vital we have an honest chat. I’d need to know if you:

  • Have ever had an allergic reaction to Alprostadil or any of its components.
  • Have any other allergies, whether to foods, dyes, or preservatives.
  • Are taking any other medications, including over-the-counter drugs or supplements.
  • Have a penile implant.
  • Have a history of priapism (a painful, long-lasting erection).
  • Have any condition affecting the shape of your penis, like Peyronie’s disease, or conditions that can affect blood flow, such as sickle cell disease, leukemia, or multiple myeloma.

How Do You Use It?

Alprostadil comes in two main forms, and how you use it is quite different for each. If we decide to proceed, my team and I will give you detailed, hands-on training right here in the office. You won’t leave until you feel completely comfortable.

Injectable Form

This is the most common and effective method. It comes as a powder that you mix with sterile water before drawing it into a very tiny needle. You then inject this small amount of liquid directly into the side of the penis (the corpus cavernosum, to be specific). It sounds a lot more intimidating than it is, I promise.

Finding the right dose is a process we’ll do together. We start low and adjust gradually based on your body’s response. It’s not a one-size-fits-all thing. Sometimes, it’s mixed with other vasodilators (in formulations called Bimix or Trimix) to boost its effect.

Intraurethral Suppository

This form involves a tiny medicated pellet that you insert into the opening of your urethra (the tube you pee through). The pellet dissolves and the medication is absorbed into the surrounding tissues.

Frankly, studies show this method is generally less effective than the injection and has a higher chance of causing pain or a burning sensation. But for some men, it’s a preferable option.

Using Alprostadil Safely: A Quick Guide

Following the rules is everything when it comes to this medication. It’s powerful, and we need to respect it. Here’s a quick summary of the most important guidelines we’ll discuss.

How fast does it work? You should expect an erection within 5-20 minutes for injections, and about 10 minutes for the suppository.
How long does it last? A typical erection from Alprostadil lasts between 30 and 60 minutes. It should go away within the hour, even if it persists briefly after orgasm.
How often can I use it? For injections, no more than 2-3 times per week, with at least 24 hours between doses. For suppositories, no more than twice in 24 hours.
Can I mix it with Viagra®? No. Absolutely not. Combining Alprostadil with oral ED drugs dramatically increases your risk of a prolonged, dangerous erection (priapism).

When to Call the Doctor or Get Help

While generally safe, there are times you need to reach out. Your safety is our top priority.

Go to the Emergency Room Immediately
Your erection is painful or lasts longer than 4 hours. This is a medical emergency known as priapism. It can cause permanent damage if not treated quickly. Do not wait.
Call Our Office As Soon As Possible
Your erection lasts longer than 2 hours but less than 4. This is a sign your dose might be too high. We need to know about it.
You notice new scarring, hard lumps, or a painful curve in your penis (fibrosis). We need to examine this to prevent long-term issues.
You feel dizzy, faint, or develop flu-like symptoms. These can be side effects like low blood pressure (hypotension) that we need to address.
You have pain, bruising, or swelling at the injection site. While some minor bruising can happen, persistent pain or swelling is not normal.
