It’s one of those moments in the clinic. You’ve been telling me about a tightness in your chest, or maybe it’s a flutter that comes and goes. Or perhaps you’re just feeling more breathless than usual. After we’ve talked it through and I’ve done an initial check-up, I might say, “You know, I think we need to get a better look at your heart.” That’s where cardiac imaging comes into the picture. It’s a bit like having a set of special cameras that let us see inside, to understand what your heart is doing and how it’s structured.
What Exactly is Cardiac Imaging?
So, cardiac imaging, or cardiovascular imaging as it’s also known, isn’t just one single thing. It’s really a group of different ways we can take pictures of your heart and the blood vessels around it. Think of it as a toolkit, and we pick the right tool for the job, depending on what we need to find out.
These pictures can tell us so much. They help us see if your heart muscle is strong, if the valves are working properly, or if there are any narrow spots in your arteries. It’s all about getting the clearest possible view of your heart’s health.
The Different Kinds of Cardiac Imaging We Use
We have quite a few ways to get these important images. Each one gives us a slightly different piece of the puzzle. Here are some of the main ones we might talk about:
Echocardiogram (Echo)
You’ve probably heard of an echocardiogram, or “echo.” This is a very common test, and for good reason! It uses ultrasound – those are high-frequency sound waves, just like the ones used to see babies during pregnancy – to create moving pictures of your heart.
- We can see your heart’s chambers, the valves opening and closing, the walls of your heart, and even the major blood vessels.
- A special type, called a Doppler echocardiogram, can show us how blood is flowing through your heart.
- It’s great for checking how well your heart is pumping (this helps us understand the severity of heart failure), and it can spot valve problems, infections, blood clots, or even a hole in the heart. Because it doesn’t use radiation, it’s a go-to test for many heart doctors.
Cardiac Computed Tomography (CT)
A cardiac CT scan is a bit more advanced. It uses a computer to combine multiple X-ray images taken from different angles. This creates incredibly detailed cross-sectional pictures, almost like slices, of your heart. We can even use these to build 3D views!
- Your doctor might suggest a CT scan if we’re looking for blockages in your arteries (often called a CT coronary angiogram) or if there might be a structural issue with your heart.
- Sometimes, we use it when other tests haven’t given us all the answers we need. Heart surgeons or interventional cardiologists (doctors who do procedures like stents) often use CT scans to map out the heart before a procedure.
Nuclear Cardiac Stress Test
This one sounds a bit intense, but it’s a really useful way to see how your heart handles work. A nuclear cardiac stress test uses a tiny, safe amount of a radioactive substance, called a tracer. We gently inject this into your bloodstream. A special camera then takes pictures of your heart, showing how well blood is flowing to the heart muscle.
- We usually do this twice: once when you’re resting, and again after your heart has been “stressed,” usually by exercising on a treadmill or, if you can’t exercise, with medication that mimics exercise.
- This test is also known as myocardial perfusion imaging (MPI).
- There are a couple of types:
- Cardiac PET (Positron Emission Tomography) scan: This type of nuclear imaging focuses on how your heart tissue is working, its metabolism. It’s good at finding damaged areas, like scar tissue after a heart attack, and can also be used for stress testing. Often, a PET scan is done along with a CT scan (a PET-CT).
- Cardiac SPECT (Single-Photon Emission Computed Tomography) scan: Similar to PET, a SPECT scan uses a tracer to show blood flow and heart tissue function. The SPECT camera detects where the tracer has been taken up by your heart muscle.
Coronary Angiogram (Heart Cath)
A coronary angiogram, sometimes called a “heart cath” or left heart catheterization, is a special X-ray test that gives us a very direct look at the arteries supplying your heart.
- Here’s how it works: a doctor, usually a cardiologist, will numb an area in your groin or wrist. They then gently guide a very thin, flexible tube (that’s the catheter) through an artery up to your heart.
- Once it’s in place, a special dye is injected through the catheter. As the dye flows through your coronary arteries, an X-ray machine takes videos. This clearly shows if there are any clogs or blockages.
Cardiac MRI (Magnetic Resonance Imaging)
A cardiac MRI is another powerful imaging tool. Instead of X-rays or radiation, it uses strong magnets and radio waves to create incredibly detailed pictures of your heart.
- It can show all the different parts of your heart – the muscle, chambers, valves, and major blood vessels – and how well they’re working.
- An MRI is fantastic for diagnosing a wide range of conditions because it can detect things like scar tissue from a previous heart attack, inflammation, or problems with the heart muscle itself. It sounds a bit like science fiction, doesn’t it, using magnets like that? But it’s very effective.
Multigated Acquisition (MUGA) Scan
This one’s a bit of a mouthful: a multigated acquisition (MUGA) scan. Its main job is to check how well the lower chambers of your heart, the ventricles (which are the main pumping chambers), are working.
- Similar to a nuclear stress test, a small amount of a radioactive tracer is injected. A special camera then takes pictures of your heart as it beats, capturing images at specific points in each heartbeat.
- The MUGA scan is particularly good at measuring something called ejection fraction. That’s the amount of blood your heart pumps out with each beat. While it’s not used as often these days, it can be very helpful for monitoring heart function, especially for patients undergoing treatments like chemotherapy that might affect the heart.
Sometimes, we might even combine these tests, like a CT scan with a coronary angiogram, or a PET scan with a CT scan. It all depends on what information we need to get the clearest picture of your heart health.
When Do We Perform Cardiac Imaging?
You might be wondering, “Why would I need one of these tests?” Well, there are several reasons we might suggest cardiac imaging:
- Screening: Sometimes, especially if you have risk factors for heart disease, we might use imaging to screen for problems before you even have symptoms. Early detection is key.
- Diagnosis: If you are having symptoms – like that chest pain we talked about, or shortness of breath, or palpitations – these tests are crucial for figuring out what’s causing them. They help us diagnose conditions like:
- Arrhythmia (irregular heartbeats)
- Coronary artery disease (narrowed or blocked arteries)
- Heart attack (to see if one occurred and how much damage there is)
- Heart failure (when the heart isn’t pumping as well as it should)
- Pediatric and congenital heart conditions (problems with heart structure present from birth)
- Heart valve disease (when one or more heart valves aren’t working right)
- Pericardial disease (problems with the sac-like lining around the heart)
- Monitoring: If you’ve already been diagnosed with a heart condition, we use imaging to see how you’re responding to treatment, or to monitor your condition over time.
Getting Ready for Your Cardiac Imaging Test
If we decide that a cardiac imaging test is right for you, we’ll go over everything you need to know to prepare. It’s usually quite straightforward.
- For tests like stress tests and coronary angiograms, we might ask you to avoid food, drinks, or medicines with caffeine for a bit beforehand.
- If you’re having a cardiac CT scan or cardiac MRI, it’s best to leave jewelry at home.
- For a cardiac MRI, because of the strong magnets, you’ll need to remove hearing aids, glasses, hair clips, or anything else with metal.
- Comfortable clothes without metal snaps or zippers are always a good idea.
And, super important: please make sure to tell us:
- About any medical conditions you have.
- All the medications you’re taking – that includes prescriptions, anything over-the-counter, supplements, and even recreational drugs.
- If you have any implanted medical devices, like a pacemaker, artificial heart valves, or metal rods or screws anywhere in your body.
- If there’s any chance you might be pregnant.

Are There Any Risks?
I know any medical test can bring a little anxiety, but cardiac imaging procedures are generally very safe. Most are non-invasive (meaning nothing enters your body) or minimally invasive (like the catheter for an angiogram).
Very rarely, some people might have an allergic reaction to the contrast dye used in tests like CT scans or angiograms. If you have a known allergy, please let us know! We can often take steps to prevent a reaction or use an alternative if needed.
